Simply login in to your account and click on App section in sidebar.and delete every apps that uses or connect with your facebook profile. otherwise if you use a Connect site to log in with your Facebook information, the account deletion will be canceled and your account will be reactivated.

Now click on  “Submit” button.  Now when it pops up a confirmation message, enter your password and validate the captcha image and click on the ‘Okay’ button and follow the rest of the instructions.
